Cytoxan code changing?

    Question Cytoxan code changing?
    Medical Coding Books
    We currently use J9096 and J9095 for Cytoxan. Has anyone heard of these codes changing recently?

    The Cytoxan has changed to J9070 which is a 100mg billable unit.
