    Default consent to treatment form

    AAPC: Back to School
    Would anyone be willing to share their consent to treatment form? My office wants to make sure they are covered especially since they see a lot of Alzheimer patients. They documented that the patient consents to and is able to participate and benefit from psychotherapy. Any suggestions would be helpful.
