I am taking my CPC test this coming Saturday, and with all my school books, I can't find how to code history of problems correctly. Can someone please refresh how to do this?

Default Look under the ICD-9-CM guidelines

Looking up history under the guidelines would give you the information you need to appropriately code history.
    Good Luck on your test!
