    Default DM w/ neuropathy

    AAPC: Back to School
    can 250.00 be coded with 356.9? or should it be coded as 250.60 and 357.2?our encounter forms do not list anything except 250.00(1,2,3). so our docs check the DM w/o mention of complications; then they check 356.9 neuropathy, peripheral unspec. what is the proper coding? is documentation needed to use 250.60/357.2? Thanks

    The doctor has to specify that it is "diabetic neuropathy" or the neuropathy is "due to" I would query the provider. If it isn't linked in the chart note by similar statements then the 250.00 and 356.9 respectively are the most correct. We cannot assume the linkage
