If the doc states patient with history of alcoholism and patient drinks 1 liter of Vodka daily can I code that as continuous use or does the doc has to specifically state continuous use?

If the MD documented he drinks 1 liter vodka qd along w/history of alcoholism then yes you can code 303.91. If not you'd have to use v11.3.
