I am looking for a good resource for auditing established patients on chemo. I have seen a couple of posts where others struggle with this also. My provider feels that any patient still on chemo should be 99215 because of the complexity of the patient with side effects and chemo planning at each visit. I was told that every visit requries the chemo to be evaluated for adjustments and side effects. While I don't disagree with the provider I feel that minor side effects and minor adjustments may be level 4 where major level 5. There just does not seem to be enough diagnoses or treatment options for an established patient to get 4+ at every visit.
Has anybody come up with a grid or coding tool for hem/onc providers? Seems like there should be a separate code fo the providers to get credit for all thier chemo management like the radiation treatment management codes.
