    Default DM coding guidelines

    AAPC: Back to School
    Good morning everyone

    In a facility setting physicians are using the software and coding themselves for their patients. When I reviewed the documentation they wrote A1C lab value and code diabetes uncontrolled (right code as per clinical evaluation) .Is it Ok for audit?

    the diabetes must be documented as uncontrolled not inferred with a lab value.
