    Default does plan for xxx indicate coding xxx???????
    Medical Coding Books
    does PLAN: TRANSFUSE PLATELETS constitue coding the transfusion without a procedure note or any other plan to do whatever else without a progress note of the plan carried out?

    no it doesnt because it only states "plan". You need documentation to support how it was done etc.
