    Can a new patient be billed as "incident to" if the physician only reviews and agrees with the treatment plan but is not involved in the History, Exam portion of the visit? How involved does the physician have to be to bill a new patient "incident to" ?

    For Medicare purposes and any payer who follows their "incident to" guidelines, a new patient can never be billed as incident to.

    Can a new problem be treated under the incident 2 guidelines?

    No a new patient can be billed by the physician using the physician's NPI or by the NPP using the NPP's NPI. A new patient can never be billed using incident -to guidelines.
