I have recently been denied, upon appeal, due to an illegible signature. I want to implement a signature log and/or attestation statement. Our physicians have plans, calculations, devices, etc. that require a signature/initial to show their participation in all phases of treatment. I have read the guidelines from the CMS manual but was wondering if anyone has implemented a log/attestation statement in the patient charts, yes, we are still paper!
I would like to review one or have some idea exactly what to include and format.
