modifier needed?

    Medical Coding Books
    Hi - Our billing department is receiving denials from Blue Cross Freedom Blue for claims billed using CPT codes: 99213, 82962 and J0570. The denial is specifically for code J0570 stating that a modifier is required. I am not familiar with this situation, and am wondering if it is an insurance specific requirement? Has anybody else has received this denial? if so, what modifier is being used? Thanks in advance for any input on this

    Columbia, MO
    You need and NDC number for this J code and you need an injection administration code such as 96372.
