Coding multiple conditions

    Question Coding multiple conditions
    Medical Coding Books
    the provider documented in the record that pt has multiple medical conditions and list all of them; my question is do I have to code all of them? is there a dx for multiple medical conditions? or just code the reason for the visit? Please help!!!!

    Does the doctor document any sort of treatment or management for the list of conditions that he/she names in the the encounter? Or is it just a laundry list of DXs? If the condition is documented with treatment/management, yes, you code them accompanied with the correct cpt code. If there is no management listed, you refrain from coding it.

    Default coding multiple conditions
    No there is no treatment plan just a list of dx. Thank you for your reply.
