    I need some help with coding a procedure. Report states;

    DX; Prolapsed iris, right eye

    OP PERFORMED; Surgical repair of prolapsed iris, right eye

    A lid speculum was carefully placed on the right side. Then 4% unpreserved lidocaine was used topically for anesthesia. A paracentesis was performed with a diamond knife and viscoelastic was used to tease the iris back into position our of the previous surgical incision. The incision was closed with two interrupted 10-0 nylon sutures and found to be water-tight and intact. The viscoelastic was irrigated out of the eye with balanced salt solution. Miostat was instilled with the conclusion of the case and the pupil became miotic in a symmetrical fashion. The lid speculum was removed. TobraDex ointment was placed on the eye. The eye was patched and shielded. The patient will be seen in followup within 24 hours.

    How would this be coded?

    We had another coder code it as 66500, the provider coded it 66250. Does it help to know it is a post cateract surgery?

    Yes your doctor is correct with the code 66250.
    The cataract operation means it involves anterior segment surgery.
    (because lens is also a part of the anterior segment just as the iris and ciliary Body does. ( though the lens does not belong to the anterior chamber, whereas the iris and ciliary body does).
    The previous surgery was in the anterior segment. The iris prolapsed after the anterior segment surgery. So its revision/repair merits the code 66250.
    No we know it for sure, what type of surgery the patient under went and the code goes for 66250.
