Default how code a piggyback IOL implant?

    AAPC: Back to School
One of our physicians says he wants to do a piggyback IOL implantation. The patient already had cataract extraction w/IOL, but now needs a 2nd IOL implanted same eye, piggyback attached. I believe the first implant will remain. Is CPT code 66985 appropriate for this or should we use unlisted. I looked on the internet and it sounds like this is done for different reasons. I just have never heard of it and would like advice on coding.

    Any help would be appreciated.

Yes, 66985 is the correct code for a Piggyback IOL. It is not done often but I have seen a few.

If you are replacing an old lens, it is not a piggyback. Use 66986 for Exchange of IOL
