    Hi friends,

    My physician is asking where to write the birth history of baby in e/m history section.
    can it be noted under social history.
    he is a ethical man so,he want to document all the birth history of baby.

    I would put birth history under past medical history because it falls under its subcategory of "previous hospitalization."
