Facial trauma

I am fairly new to ENT coding. I feel that I am doing fairly well until I get a Facial trauma operative note. I am currently trying to get some continuing education in maxillofacial but in the mean time I need help. I know everyone is busy and it bothers me to ask, but, is there anyone that could take a look at a few OP notes and let me know if I am even in the ballpark?

Default Post the scrubbed note with your best effort
Post the scrubbed op note here (or in the General Surgery forum) .... ONE note per POST, please

INCLUDE your best effort at coding and any specific questions you have about modifiers or bundling.

    Hope that helps.
