I'm seeking advice/ideas/best practice for capturing a provider's "out of office" charges, meaning things like ER or Inpatient consults, surgeries, or anything else done outside of the practice office. I am a hospital med rec manager who now oversees the pro fee coding of our clinic providers, and we are trying to make sure we have captured all applicable charges.

Does anyone have a charge ticket or other document that you use to capture this information that you could share? We have the in office encounters down pat, but struggle with missing encounters done external of the office.
