If we saw the pt on Friday 99213 and then the patient brought in a urine sample on the following Monday .. should we bill the U/A as part of Friday's visit ..or should we bill a 99211 + 81002 on Monday ?

you cannot bill a 99211 for a patient to bring in the ua sample. If you run the test on Monday then bill only the 81002 on Monday

    Default 99211 u/a
ok, the U/A was performed and the findings were discussed with the patient (spending 5 minutes discussing this).. then wouldn't the 99211 would be justified ?

Only if the findings were other than normal and it was discussed by the physician with patient and not the nurse with the patient.

The findings were abnormal and the nurse discussed this with the pt and per the CPT book: a 99211 states the presence of a physician may not be required, so wouldn't that suffice?

It does not qualify as it is not a follow up visit for something already defined by the physician.
