Default Billing for completion of Disability Forms

    AAPC: Back to School
Has anyone sent in claims using 99080?? And how is the reimbursement? If any? My Physicians are asking if there are any codes that we may use to bill for forms that need to be completed. Such as Disability Forms. Thanks
