    The health system wants the provider to bill the health system on a HCFA for physician record review. All charts in question involve patients that
    Are health system employees
    Were previously seen at employee health
    Had no face to face by reviewing physician
    The reviewing physician reviews labs, rads, other employee health chart documents to validate patient's suitability to return to work or need for more face to face, more care, more time away, etc.
    This charge does not include additional face-to-face encounters If the patient is called back in.
    Unlisted. 99499?
    99455, 99456 99080 aren't good fits. What do you think it is?

    I would go with 99358 as long as time is documented and it is 30 minutes or more.
