    Procedure: CT-guided left L5 nerve root block.

    Medications: 1% lidocaine local anesthesia, 1.0 ml 0.25% bupivacaine ,
    1.0 ml Kenalog (40 mg/ml)

    Complications: None.


    After the procedure, including indications and potential complications,
    had been discussed with the patient and questions answered, written
    informed consent was obtained. A time out was performed. The patient was
    then taken to the CT suite and placed on the CT table in prone position
    with the skin of the back was prepped and draped in the standard sterile
    fashion. Local anesthesia was achieved with 1% lidocaine. Under
    intermittent CT guidance a 22-gauge spinal needle was advanced to the
    soft tissues of the back until the tip of the needle resided adjacent to
    the left L5 nerve root. 1.0 ml 0.25% bupivacaine and 1.0 ml Kenalog (40
    mg/ml) were injected adjacent to the nerve root and the needle withdrawn.
    The patient tolerated the procedure well without immediate complication
    and was discharged in stable condition after a period of observation.

    Pre-procedure pain of 10/10 was noted to be 5/10 immediately following
    the procedure. The patient was given instructions for followup and was
    told that multiple treatments may be necessary to alleviate her back/leg

    Technically successful left L5 nerve root injection as above.

    Probably, but he doesn't say transforaminal so you need to ask him

    i agree with 64483. i was told that some doctors might say transforaminal and some may say nerve root or nerve root block. so pretty much nerve root block=transforaminal injection
