    Default 25 mod on exam/ same diagnosis

    Have any of you seen where it isn't necessary to have a different diagnosis on an exam and a procedure when using a modifier 25 anymore? I want to say I've seen this recently but I can't be sure where and by whom.

    It is in the definition of modifier -25 in CPT appendix A.
