    Default Bleeding

    AAPC: Back to School
    How do you know which diagnosis to use. Doctor states abnormal uterine bleeding, dysfunctional uternie bleeding and menorrhagia. I'm guessing he is documenting all three because he isn't sure what the source of bleeding is.

    You should NEVER guess - query the provider? Make sure he documents accurately.
