    Medical Coding Books
    I do coding for Home care and wanted to know if the nurse (RN) gets verbal diagnosis information from the MD can I use that to code or do I have to have the MD provide signed documentation of the diagnosis?

    Remember, if it's not documented, it was not done. Get it documented.
