Preventive with modifier 25

    Wink Preventive with modifier 25
    Medical Coding Books
    If patient comes in for annual exam and also gets immunizations would you use modifier 25 on the preventive visit?

    Ex: 99396
    90734
    90471

    99396-25 v70.0
    90737 v03.9
    90471 v03.9

    Or do you use the mod. 25 just on E/M codes for injections and immunizations

    hi kimb - yes, I use a .25 on the preventive when an immunization is given. As long as documentation supports the charges (The preventive codes are "E/M" codes as well as the "level" codes and consult codes too)

    I agree with Donna. If you don't use the -25, either the admin or the preventive will be denied.
