Can 95165 (preparation of serum) be billed with 99214, (office visit)? If so does that also apply to Medicare?

If there is a significant, separately identifiable service performed on the same day, you can bill an E/M code and append modifier 25 to the E/M code as long as the documentation supports this service. The separate service should be unrelated to the preparation of the serum since the physician work in doing that is covered in the 95165 code.
