Default Using Meds as Modifying Factor

    AAPC: Back to School
I was re-reading through some threads and also other web sites, and want to know what others think.

Pertinent to the HPI, when a medication is only stated and doesn't mention whether it makes the problem better, worse, stable, etc. Would you count the med as a modifying factor?

i.e. pt here f/u hypothyroidism, diagnosed in 2005. c/o increasing appetite, on Tapazole 40 mg.

I have read where some people say if it doesn't state what the med does, it would fall under current meds, not HPI.

I agree that it must be stated what the med does or it doesn't count as a modifying factor.
