    Default children being followed due to low birth weight

    We have a clinic that assesses and follows children who were born prematurely (usually fell into the 765.0x and 765.1x categories at birth). Many times they do not currently have problems, but are being followed as a precaution. Any suggestions as to the correct diagnosis code to use? Personal history of???
