we are seeing issue with bcbs of tn and patients that originate to the providers offic for a sceening colon. When the physician does the colonoscopy and finds a polyp and removes the polyp we would code the procedure with: 453xx v7651, 2113 with 453xx linked to 2113.
Should i not link the codes together??

you should link both dx codes to the procedure and use the 33 modifier on the procedure.
