Default need advice for infusions 96365 and 96367

There are those of you amongst us who are experienced with these codes, and I need some advice. The established patient is seen in the specialist's office and has requested acute service. Here's an extract: " I advise the patient to undergo infusion of caffeine sodium benzoate. About 500 mg of caffeine sodium benzoate was diluted in l L and infused over five hours. IV infusion started at 4 PM and ended at 9PM. Patient still had residual headaches. An injection of morphine 20 mg IM was given. Pt was monitored for 30 minutes. IV was discontinued and removed and patient was discharged....etc."
Question: 5 hours of continuous infusion. How would this lengthy process be coded? Also, thinking this might be a case where an extended service would be appropriate, but basically need the doctor to notate how much time he actually spent with the patient! Can I get some comments? Also, patient has commerical insurance carrier.

96365 x 1 1st hour 4-5p
96366 x 4 add'l hours same substance 5-9p

96372 x 1 intramuscular injection
