Default Billable Amounts

I am trying to help a physician set up billable amounts to go out to the insurance companies. I thought I read somewhere that you had to charge a certain percentage over the allowable amount that was the write off amount. Is this true or how is the billed amount determined if this is not true? Thanks in advance for your repsonses.
