7 years ago the HITECH Act authorized incentive payments to increase Electronic Health Record (EHR) adoption. This financial inducement was successful with 78% of practices utilizing EHR systems in 2013, up from just 18% in 2001. With this significant shift to electronic records, for many practices, came the unintended consequence of physicians/providers selecting diagnosis codes by number (ICD-9 and then ICD-10-CM). Physicians/providers were untrained in the use of the International Classification of Disease (ICD) system, so this transition was and continues to be fraught with problems.
