I wanted to confirm if this is accurate. When a Toxicology lab receives a specimen, along with the Requisition Form with the tests marked off to be tested, shouldn't a DX be on the form, as a requirement to run the test? So basically we get Req's with no DX and no provider or pt signature, on them. I was always taught you have to have a dx to run a tests, and of course it be signed.

Accompanying the Req, is also a face sheet with DX and that is where the DX is being selected from, but that is a facesheet and not actual medical records documentation.
