Question: How should we bill for J codes? Should they be billed with revenue center codes 636 or 250?

Answer: HCFA states that J codes (injected drugs) associated with packaged drugs should be billed using revenue code 250, and J codes used to apply for pass-through payment should be billed using revenue code 636. The question still remains, however, as to how to bill J codes that are packaged. The most prudent advice is check with your fiscal intermediary. If it allows billing packaged J codes under revenue center 636, then put all packaged and pass-through J codes in revenue center 636 for billing.
