2 Sites Justify 2 Codes

Question: One of the doctors in my practice performed two Mohs surgeries on two different sites. He repaired both sites with porcine graft. Should I report 15400 once or twice?

Answer: Because CPT doesn't specifically state "each site" in its description of 15400 (Application of xenograft, skin; 100 sq cm or less [application of a biologic wound dressing, thin meshed skin, or porcine graft following excision of the burn wound and/or surgical preparation of the site]), you can report 15400 twice for the two sites.
