Reader Question:

Expand Keratoderma Choices in ICD-10

Question: Our surgeon excised a skin lesion which the pathology report diagnosed as a “porokaratotic lesion.” What is the correct diagnosis code for this?

Answer: Currently, you should report a porokaratotic lesion using ICD-9 code 701.1 (Keratoderma acquired).

The areas of hyperkeratosis can occur at different sites and with specific characteristics that clinicians may identify as different conditions, such as disseminated superficial actinic porokeratosis (DSAP), or porokeratosis palmaris et plantaris disseminata (PPPD).

When ICD-10 goes into effect on Oct. 1, 2015, you will need to know the more specific diagnosis in order to select the proper code. For instance, you would code PPPD as L85.1 (Acquired keratosis [keratoderma] palmaris et plantaris), but DSAP as L56.5 (Disseminated superficial actinic porokeratosis).
