MAC: You're Responsible For Physician Documentation

Don't think you're off the hook for medical record submission if those records are at your patients' doctor's office. "National Government Services expects that the patient's medical records will reflect the need for the medical care provided," the HHH MAC subcontractor says in an e-mail message to providers. "The supporting documentation includes any third party documentation, such as the treating physician's documentation, that may be available only through the physician's office." And make sure your physician signatures are valid, NGS urges. "A signature key or attestation statement is needed if signature is not legible."
