Medicaid Provider Files For Bankruptcy

Crystal Care Home Health Services in Richfield, Minn., has filed for bankruptcy, reports the Star Tribune newspaper. The state was investigating the agency that has 800 employees and 900 clients for Medicaid fraud, say news reports. Healthstar Home Health of North St. Paul took over Crystal Care’s clients after the fraud investigation was announced.
