Use Finalized HIS Form For Preparation

You’ve finally got an official final version of the new Hospice Item Set that goes into ef-fect July 1. It actually contains no changes from the pre-Office-of-Management-and-Budget-approved version, CMS confirms. The form is in the “Down-loads” section at www.cms.gov/Medicare/Quality-Initiatives-Patient-Assessment-Instruments/Hospice-Quality-Reporting/Hospice-Item-Set-HIS.html.
