Cut Out Excised Infection Code

Question: We are caring for a patient who had surgery to excise infected osteomyelitic bone. The osteomyelitis is a manifestation of his diabetes. How should I code for his wound care? Is it considered aftercare for surgery or is this similar to incision and drainage of an abscess and not considered surgical aftercare?

Answer: If the infection is gone, then you shouldn't code the osteomyelitis; however the bone changes are still there.
To code for this patient, report:

• M0230a: V58.78 (Aftercare following surgery of the musculoskeletal system, NEC);
• M0240b: 250.80 (Diabetes with other specified manifestations, type II or unspecified type, not stated as uncontrolled);
• M0240c: 731.8 (Other bone involvement in diseases classified elsewhere);
• M0245a: 250.80 (Diabetes with other specified manifestations, type II or unspecified type, not stated as uncontrolled); and
• M0245b: 731.8 (Other bone involvement in diseases classified elsewhere).
