Know What To List For Diabetic CKD

Question: Our patient has chronic renal failure due to diabetes, CHF, and urinary retention. In column two, we listed the following codes:

250.40 (Diabetes with renal manifestations; type II or unspecified type, not stated as uncontrolled);

585.3 (Chronic kidney disease, Stage III [moderate]);

428.0 (Congestive heart failure, unspecified); and

788.20 (Retention of urine, unspecified).

Is that correct?

Answer: Your answer is correct as long as you have documentation that the chronic kidney disease (CKD) is stage 3. If the CKD is due to diabetes, then you should code for the diabetes with the fourth digit to indicate renal manifestations.

Next, list the renal manifestation code (585.3).

You should also code other conditions that require active intervention or may impact the patient's care, so list the CHF and urinary retention.
