When Is It too Late for Aftercare?

Question: Our new patient was referred to home health by a facility. Back in July she fractured her pelvis and also had a pin inserted for a hip fracture. Our agency is providing therapy at home to deal with the healing fractures. We are also

sending a nurse to address some cardiac issues by checking weights and medications. Can we still use an aftercare code for the fractures since they happened so long ago?

Answer: There is no time limit to aftercare. The aftercare includes the rehabilitation and any other treatment adjustments, so the aftercare of the fracture code is appropriate. Also consider that the aftercare of fracture codes now provide risk

adjustment for certain outcomes.
