Ignore These Pharmacists Reminders At Residents Peril

Examples of important reminders from consultant pharmacists that physicians ignore at their patients peril include the following, according to certified medical director Duncan MacLean:
 
 Drug-drug interactions with Coumadin (may prolong the prothrombin time);

 Hyperkalemia risk from ACE inhibitors used in patients with renal insufficiency;

 Dilantin dosing in hypoalbuminemia (less protein binding, causing toxicity at a normal blood level).
