Should the SNF do a Sig Change MDS Assessment in This Case?

Question: If we have done the 5-day PPS assessment on a resident who goes back to the hospital and is readmitted to the SNF, how do we proceed with the next MDS assessment, if the person has undergone a significant change in condition?

Answer: The answer depends on whether you completed and submitted the OBRA initial assessment combined with the 5-day PPS assessment when the resident was discharged. You shouldn't do a significant change in status assessment until the initial OBRA assessment has been accepted into the database. If you didn't complete and submit the initial assessment, and instead submitted a discharge tracking form indicating that the resident was discharged before the initial OBRA assessment was completed, then you'd do the initial assessment combined with the readmission Medicare assessment or the 14-day Medicare assessment.
