Mrs. P received the following medications during the 7-day lookback:
Haldol 0.5 mg po BID PRN: Received once a day on Monday, Wednesday, and Thursday
Ativan 1 mg po Q AM: Received every day
Restoril 15 mg p.o. Q HS (at bedtime) PRN. Received at bedtime on Tuesday and Wednesday only
Mrs. P became severely short of breath in the middle of the night during the last seven days. She was transferred (but not admitted) to the emergency room (ER) at the local hospital. Upon her return to the nursing facility, the ER transmittal record stated that she had received 1 dose of IV Lasix.
