Example No. 1: The resident received heavy assistance of two persons to transfer on/off toilet. He was able to bear weight partially, and required only standby assistance with hygiene (e.g., being handed toilet tissue or incontinence pads).

Example No. 2: An obese, severely physically and cognitively impaired resident receives a mechanical lift for all transfers to and from her bed. It is impossible to toilet her and she is incontinent. Complete personal hygiene is provided at least every two hours by two persons.
