Neurology & Pain Management Coding Alert

Reader Question:

Sympathetic Skin Potential Testing

Question: Can one bill two units of 95923 for bilateral (foot) testing of sympathetic skin potential? Should modifier -50 be used?

Answer: According to the Medicare fee schedule, the 150 percent adjustment for bilateral services does not apply. Even if you file with a -50 modifier or -RT and -LT modifiers, the fee will be limited to 100 percent of a single code. The bilateral adjustment is inappropriate for codes in this category because of either physiology or anatomy, or because the code description specifically states that the service is unilateral and there is a bilateral code.
