Question: How should we code a costovertebral joint injection?Maryland SubscriberAnswer: The costovertebral joint is considered a small joint, so you should report 20605 (Arthrocentesis, aspiration and/or injection; intermediate joint or bursa [e.g., temporomandibular, acrominoclavicular, wrist, elbow or ankle, olecranon bursa]).Description: Costovertebral joints lie between the ribs and vertebrae (compared to costochondral joints that connect the ribs to the sternum). If physicians administer the injection while using fluoroscopy for needle guidance, include 77002 (Fluoroscopic guidance for needle placement [e.g., biopsy, aspiration, injection, localization device]) on the claim.
