Subcutaneous Reservoir

Question: What is the proper code to report removal of a subcutaneous reservoir? Virginia Subscriber Answer: Implantation of a subcutaneous reservoir is reported using 61215 (Insertion of subcutaneous reservoir, pump or continuous infusion system for connection to ventricular catheter) but does not include removal. And CPT does not designate a specific code for removal of a subcutaneous reservoir. Therefore, the best code choice is 64999 (Unlisted procedure, nervous system). Whenever submitting an unlisted-procedure code, be sure to include a copy of the operative report with the claim so the insurer can make an informed payment decision. Also, some payers may consider payment for the removal to be included in the placement (61215) when performed by the same physician.
