Alabama SubscriberAnswerCodes 22630 (Arthrodesis, posterior interbody technique, including laminectomy and/or discectomy to prepare interspace (other than for decompression), single interspace; lumbar) and 22612 (Arthrodesis, posterior or posterolateral technique, single level; lumbar (with or without lateral transverse technique)) are two distinct and separately identifiable procedures and can be billed together with the appropriate modifier (-59), even when performed at the same lumbar level.
